What Must be Turned In
- Registration form (1601) must be properly filled out
- Must have proof of age - Birth certificate, Passport, Green Card - No hospital certificates
- All cards for coaches and players must have a clear picture without hats or sunglasses, signatures are not required this year.
- All coaches must fill out The Coach Registration Form (1628) If anyone answers yes to the felony charges that form must be brought to the office within 24 hours.
- We will be doing player/coach passes for U12's & up.
- You will be charged for players based on the report provided by leagueone. This year is a flat fee per player.
- Follow the guidelines for team formation numbering and naming in the powerpoint.
DEADLINES
- Sept 1 2007 - TEAMS MUST BE FORMED IN LEAGUEONE SYSTEM. Teams not yet formed may not be scheduled.
- Sept 4 2007 - Payment for players registered to be into office by 5PM
- Sept 20 2007 - Final Roster movement AFTER THIS DATE I will be "APPROVING" Teams and CYSA transfers will be required.
WHAT NEEDS TO BE TURNED IN:
- "contact list" roster with 1628's for all coaches involved with the team.
- Payment due to the office Sept 4 by 5PM.
- You can print these roster through the print roster option when you are in MAINTAIN TEAMS for each team.
